Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the key thing to understand: this interaction is really about the thyroid extract, not your cancer medicine. The concern on file is that other pills and foods can block thyroid hormone from being absorbed. So when you take your osimertinib (Tagrisso) at the same time as the thyroid extract, it's the thyroid hormone that may not absorb well, which can leave thyroid symptoms (tiredness, feeling cold, weight changes) undertreated.

Osimertinib itself is not a prodrug, and there's no strong sign it needs to be activated by your thyroid hormone. The simple fix is timing: thyroid products work best on an empty stomach, taken apart from other medicines. Please run your full list past your oncology pharmacist so they can space these properly and confirm nothing else is missed.

About Osimertinib

What Is Osimertinib Used For?

Brand name: Tagrisso

Tagrisso is used to treat adults with non-small cell lung cancer (NSCLC) whose tumors have specific EGFR mutations — either an exon 19 deletion or an exon 21 L858R mutation — confirmed by an FDA-approved test. It is used in several settings: after surgery to reduce the risk of cancer returning (adjuvant therapy), for locally advanced stage III NSCLC that has not progressed after chemoradiation, and as a first-line treatment for locally advanced or metastatic NSCLC (including in combination with pemetrexed and platinum-based chemotherapy).

Tagrisso is also approved for adults with metastatic NSCLC that carries a specific resistance mutation called T790M, when the cancer has stopped responding to another EGFR-targeted therapy.
