Penicillamine falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the concern runs in the opposite direction from how you might expect. Penicillamine is a copper-binding medicine, and it doesn't just grab onto copper. It can also latch onto other minerals and substances in your gut. When that happens, the thyroid extract can become the one that loses out, because penicillamine and the leftover minerals in the gut may bind up the thyroid hormones (T3 and T4) and keep them from being fully absorbed.

This is a minor, timing-related issue, not a dangerous one. The simple fix is spacing them apart, ideally taking each one a few hours from the other. Since over-the-counter thyroid extract is unpredictable to begin with, it's worth asking your pharmacist or doctor to help you sort out the timing and confirm you actually need it.

What Is Penicillamine Used For?

Brand names: Cuprimine, Depen Titratable

Penicillamine is used for three distinct conditions. It treats Wilson's disease — a rare genetic disorder in which the body accumulates dangerous levels of copper — by binding to the excess copper and helping the body remove it. It also treats cystinuria, a condition where the amino acid cystine builds up and forms painful kidney stones, by converting cystine into a more soluble form that the body can excrete more easily.

For patients with severe, active rheumatoid arthritis who have not responded adequately to conventional therapies, penicillamine (available as Aagylur, Cuprimine, Depen, and other products) may be used to help suppress disease activity and reduce joint pain, tenderness, and swelling.
