Pyrantel Pamoate falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the good news: this interaction is a minor one. When you take a thyroid extract by mouth, it absorbs best on an empty stomach because other things in your gut can grab onto the hormone and block it. Pyrantel pamoate (Reese's Pinworm) is actually a nice partner here because it barely gets absorbed at all. It stays in your intestine to paralyze the pinworms, so it isn't really competing for the same absorption your thyroid hormone needs.

To play it safe, I'd simply space them out and take your thyroid extract on its own, ideally first thing in the morning before food or other products. Pyrantel is usually a one-time or short course anyway, so any overlap is brief. Run your timing past your pharmacist if you're unsure.
