Remifentanil falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the good news: this particular interaction really doesn't apply to Remifentanil. The concern on file is about oral absorption, since foods and minerals can bind to swallowed thyroid hormone and keep your gut from soaking it up. Remifentanil isn't a pill you swallow. It's a fast-acting opioid given through an IV during anesthesia or surgery, so there's no stomach absorption step for thyroid extract to get in the way of.

Remifentanil is also broken down by enzymes in your blood and tissues, not by the liver pathways that supplements usually mess with. So thyroid extract isn't expected to change how it works. Still, because thyroid extract affects your metabolism and heart rate, always tell your anesthesia team and pharmacist about every supplement you take before any procedure.
