Why this applies to Nusinersen Sodium

Nusinersen Sodium falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Tocotrienols interacts with.

Concomitant use of tocotrienols with anticoagulant or antiplatelet agents might increase the risk of bleeding. However, this has not been reported in humans.
Taking tocotrienols orally inhibits experimentally-induced platelet aggregation in humans. Theoretically tocotrienols might increase the risk of bleeding if taken with antiplatelet or anticoagulant drugs. However, tocotrienols 400-800 mg daily have been used with aspirin and/or clopidogrel for 1 year with no clear cumulative antiplatelet effects and no reports of bleeding.

I want to be straight with you here. The interaction in our database was written for blood thinners and antiplatelet drugs, but Spinraza (nusinersen) is not one of those. It is given as a spinal injection to boost a protein your nerves and muscles need, and it does not thin your blood or work through the same pathways. So the bleeding concern listed really does not apply to nusinersen itself.

That said, tocotrienols in higher supplement doses (400 to 800 mg daily) have a mild effect on platelets in some studies, though no real bleeding was seen even alongside aspirin. Since Spinraza involves a spinal tap, anything affecting platelets is worth a quick mention. Please check with your neurology team and pharmacist before adding tocotrienols, just so everyone is on the same page around your injection days.

Brand name: Spinraza

Spinraza (nusinersen) is used to treat spinal muscular atrophy (SMA) — a genetic disease that causes progressive muscle weakness by reducing the body's ability to make a protein called SMN (survival motor neuron). Spinraza is approved for pediatric and adult patients with SMA.

Spinraza is given by intrathecal injection — directly into the fluid surrounding the spinal cord — so the medicine can reach the central nervous system where it's needed most.
