Why this applies to Iloprost

Iloprost falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Tocotrienols interacts with.

Concomitant use of tocotrienols with anticoagulant or antiplatelet agents might increase the risk of bleeding. However, this has not been reported in humans.
Taking tocotrienols orally inhibits experimentally-induced platelet aggregation in humans. Theoretically tocotrienols might increase the risk of bleeding if taken with antiplatelet or anticoagulant drugs. However, tocotrienols 400-800 mg daily have been used with aspirin and/or clopidogrel for 1 year with no clear cumulative antiplatelet effects and no reports of bleeding.

Here's the simple version: iloprost (Ventavis) does more than open up the blood vessels in your lungs. Like its relatives, it also makes your platelets a little less sticky, so it has a mild blood-thinning side to it. Tocotrienols, a form of vitamin E, have shown a slight platelet-slowing effect of their own in some studies. Put them together and, in theory, you could nudge your bleeding risk up a notch (think easier bruising, nosebleeds, or gums that bleed when you brush).

The good news is that this is a minor and unlikely concern. In studies, people took fairly high tocotrienol doses alongside other blood thinners for a year without clear bleeding problems. Still, since iloprost itself affects platelets, it's worth a quick chat with your pharmacist or doctor before adding tocotrienols.
