Why this applies to Mirtazapine

Mirtazapine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Valerian interacts with.

Valerian does not seem to have a clinically relevant effect on levels of drugs metabolized by CYP2D6.
Although some in vitro evidence suggests that valerian affects CYP2D6, clinical pharmacokinetic (PK) studies show that valerian is unlikely to affect the CYP2D6 enzyme. In one PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days did not affect the metabolism of dextromethorphan, a CYP2D6 substrate. In another PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of debrisoquine, an accepted CYP2D6 probe-substrate.

Good news here: when it comes to how your body breaks down mirtazapine, valerian doesn't seem to get in the way. Mirtazapine is processed partly by a liver enzyme called CYP2D6, and while early lab dishes hinted valerian might block that enzyme, the actual human studies tell a different story. People taking valerian for two to four weeks showed no real change in how that enzyme worked. So valerian is unlikely to make mirtazapine build up or wear off faster.

The one thing I'd gently mention is that both can make you sleepy. Valerian is used as a sleep aid, and mirtazapine is famously sedating too, so stacking them could leave you extra groggy or foggy the next morning. That's worth a quick chat with your pharmacist or doctor before you combine them.

Valerian does not seem to have a clinically relevant effect on levels of drugs metabolized by CYP3A4.
Although some in vitro evidence suggests that valerian extract might inhibit or induce CYP3A4, clinical pharmacokinetic (PK) studies show that valerian does not have a clinically significant effect on the CYP3A4 enzyme. In one PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of midazolam, an accepted CYP3A4 probe-substrate. In another PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days modestly increases levels of alprazolam, a CYP3A4 substrate, suggesting mild inhibition of CYP3A4. However, this mild inhibition is unlikely to be clinically relevant.

Here's the good news on combining valerian with your mirtazapine (Remeron): the metabolism side of this pairing is pretty quiet. While early lab tests hinted that valerian might slow down the CYP3A4 enzyme that helps clear mirtazapine, real-world human studies showed only a tiny effect that isn't considered clinically meaningful. Mirtazapine is the active drug itself, not a prodrug, so you don't have to worry about it being switched off or supercharged here.

The bigger thing to keep in mind is overlap, not chemistry. Mirtazapine already tends to make people drowsy and groggy, and valerian is a calming sleep aid, so together they may make you sleepier than expected. Please check with your pharmacist or doctor before adding it.

Mirtazapine tablets, mirtazapine orally disintegrating tablets, Remeron, and RemeronsолTab are all indicated for the treatment of major depressive disorder (MDD) in adults. Major depressive disorder involves a persistently low or sad mood most days, often accompanied by loss of interest, sleep changes, fatigue, and difficulty concentrating.

The effectiveness of mirtazapine tablets in maintaining a response for up to 40 weeks after an initial treatment period has also been demonstrated. Patients should be reassessed periodically to determine whether continued treatment is needed.
