Letrozole, Ribociclib Succinate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Vitamin E interacts with.

Theoretically, vitamin E might induce metabolism of CYP3A4, possibly reducing the levels CYP3A4 substrates.
Vitamin E appears to bind with the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of vitamin E and other drugs affected by these enzymes.

Your Kisqali Femara combo (ribociclib plus letrozole) is cleared by a liver enzyme called CYP3A4. The concern here is mostly on paper: in lab studies, vitamin E may gently speed up that enzyme. If that happened in real life, your body could break these medicines down a little faster, which in theory might lower their levels and make your cancer treatment slightly less effective. Neither drug is a prodrug, so faster clearance would mean less benefit, not more side effects.

Honestly, this is a weak, theoretical worry and has not been proven in people. Normal food amounts of vitamin E are not a concern. Before starting any high-dose vitamin E supplement, just check with your oncologist or pharmacist so they can keep an eye on things.
