Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Vitamin E interacts with.

Theoretically, vitamin E might induce metabolism of CYP3A4, possibly reducing the levels CYP3A4 substrates.
Vitamin E appears to bind with the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of vitamin E and other drugs affected by these enzymes.

Here's the short version on pairing vitamin E with rilpivirine (Edurant). Your body clears rilpivirine using a liver enzyme called CYP3A4. In lab-based theory, high-dose vitamin E may nudge that enzyme to work a little faster, which could lower rilpivirine levels in your blood. Rilpivirine is an active drug (not a prodrug), so lower levels could mean it works less well against HIV.

I want to be honest about the strength here: this is theoretical, based on how the chemicals behave, not on real patient cases. With HIV medicine, though, keeping a steady, effective level really matters for keeping the virus controlled. Everyday food amounts of vitamin E are very unlikely to be a problem. Before starting a high-dose vitamin E supplement, just check with your pharmacist or doctor first.

About Rilpivirine

What Is Rilpivirine Used For?

Brand names: Edurant, Edurant Ped

Rilpivirine tablets (sold as Edurant, Edurant Ped, and generic rilpivirine) are used along with other antiretroviral medicines to treat HIV-1 infection in patients who have not previously been treated with HIV medicines (treatment-naïve). Eligibility depends on age, body weight, and the level of virus in the blood at the start of treatment.

Edurant tablets are also approved, in combination with cabotegravir (Vocabria), for short-term oral use in adults and adolescents who are already virologically suppressed — specifically as a lead-in before starting the long-acting injectable regimen Cabenuva, or as a bridge if a Cabenuva injection dose will be missed.
