St. John's wort is the supplement I would rule out first for anyone taking rilpivirine (Edurant) for HIV. It speeds clearance of this drug class, and low levels can mean treatment failure and lasting viral resistance. That danger sets the tone for the whole page. Eleven of the 175 interacting ingredients rate major, and 121 work through CYP3A4, the liver enzyme that clears rilpivirine, so they can push its blood level up or down. Grapefruit inhibits that enzyme strongly enough to rate major on its own.
