Rilpivirine falls under QT INTERVAL-PROLONGING DRUGS — the group Iboga interacts with.

Theoretically, iboga may have an additive effect with drugs that prolong the QT interval.
Numerous case reports suggest that ibogaine, a constituent in iboga, can cause QT interval prolongation.

Here's the concern with combining Iboga and rilpivirine (Edurant). Both can affect the heart's electrical timing, something doctors call the QT interval. Rilpivirine, especially at higher than recommended doses, has a known tendency to stretch out that interval, and Iboga (through ibogaine) has been linked in case reports to the same effect. Stacking the two could add together and raise the risk of a dangerous, irregular heartbeat.

To be clear, this isn't about one drug breaking down the other. It's an additive effect on the heart. The evidence here is anecdotal and partly theoretical, but the stakes are high since serious rhythm problems can happen. Iboga also carries real safety risks on its own and is illegal in the U.S. Please talk with your doctor or pharmacist before mixing anything with your HIV treatment.

Iboga is a Central African shrub whose root bark contains ibogaine, a powerful hallucinogenic compound studied mainly for addiction. It carries serious safety risks, including dangerous heart rhythm changes and deaths, and ibogaine is illegal in the United States and many other countries. We do not recommend its use outside of tightly controlled research settings.

About Rilpivirine

What Is Rilpivirine Used For?

Brand names: Edurant, Edurant Ped

Rilpivirine tablets (sold as Edurant, Edurant Ped, and generic rilpivirine) are used along with other antiretroviral medicines to treat HIV-1 infection in patients who have not previously been treated with HIV medicines (treatment-naïve). Eligibility depends on age, body weight, and the level of virus in the blood at the start of treatment.

Edurant tablets are also approved, in combination with cabotegravir (Vocabria), for short-term oral use in adults and adolescents who are already virologically suppressed — specifically as a lead-in before starting the long-acting injectable regimen Cabenuva, or as a bridge if a Cabenuva injection dose will be missed.
