Rilpivirine falls under HEPATOTOXIC DRUGS — the group Hemp Agrimony interacts with.

Hemp agrimony contains hepatotoxic pyrrolizidine alkaloids (PA). Theoretically, using hemp agrimony with hepatotoxic drugs might have additive adverse effects on the liver. Some drugs that can adversely affect the liver include acetaminophen (Tylenol and others), amiodarone (Cordarone), carbamazepine (Tegretol), isoniazid (INH), methotrexate (Rheumatrex), methyldopa (Aldomet), fluconazole (Diflucan), itraconazole (Sporanox), erythromycin (Erythrocin, Ilosone, others), phenytoin (Dilantin), lovastatin (Mevacor), pravastatin (Pravachol), simvastatin (Zocor), and many others.

Here's the concern in plain terms: hemp agrimony can contain pyrrolizidine alkaloids, which are natural compounds known to stress the liver. Rilpivirine (Edurant) is also processed by the liver and, on its own, can occasionally raise liver enzymes. Putting the two together could, in theory, add up to more strain on your liver than either one alone.

I want to be honest that this is a theoretical caution based on how these things behave, not proven cases with rilpivirine specifically. Still, keeping your HIV treatment safe matters. Signs to watch for would include yellowing of the skin or eyes, dark urine, belly pain, or unusual tiredness. Because hemp agrimony isn't recommended internally anyway, please check with your pharmacist or doctor before combining them.

Hemp agrimony is a traditional European herb that has been used for digestion, colds, and as a general tonic, but there is very little reliable human research to support these uses. More importantly, the plant can contain pyrrolizidine alkaloids that may harm the liver, so most experts do not recommend taking it internally. About Rilpivirine

What Is Rilpivirine Used For?

Brand names: Edurant, Edurant Ped

Rilpivirine tablets (sold as Edurant, Edurant Ped, and generic rilpivirine) are used along with other antiretroviral medicines to treat HIV-1 infection in patients who have not previously been treated with HIV medicines (treatment-naïve). Eligibility depends on age, body weight, and the level of virus in the blood at the start of treatment.

Edurant tablets are also approved, in combination with cabotegravir (Vocabria), for short-term oral use in adults and adolescents who are already virologically suppressed — specifically as a lead-in before starting the long-acting injectable regimen Cabenuva, or as a bridge if a Cabenuva injection dose will be missed.
