Fosaprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Hemp Agrimony interacts with.

Hemp agrimony contains hepatotoxic pyrrolizidine alkaloids (PA) which are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the key thing to understand about this pairing. Hemp agrimony naturally contains pyrrolizidine alkaloids, which are compounds that can be hard on the liver. Those alkaloids get processed by an enzyme called CYP3A4. The concern is that drugs which speed up (induce) CYP3A4 could make your body convert more of those alkaloids into toxic, liver-damaging byproducts. So this isn't about your medication getting weaker or stronger. It's about the herb potentially becoming more harmful.

Now, Fosaprepitant (Emend) is a mild CYP3A4 inducer, but honestly, this whole interaction is theoretical and based only on lab data, not real people. The bigger picture is that most experts already advise against taking hemp agrimony internally because of those liver toxins. Please check with your pharmacist or doctor before using any product containing it.
