Naltrexone falls under HEPATOTOXIC DRUGS — the group Hemp Agrimony interacts with.

Hemp agrimony contains hepatotoxic pyrrolizidine alkaloids (PA). Theoretically, using hemp agrimony with hepatotoxic drugs might have additive adverse effects on the liver. Some drugs that can adversely affect the liver include acetaminophen (Tylenol and others), amiodarone (Cordarone), carbamazepine (Tegretol), isoniazid (INH), methotrexate (Rheumatrex), methyldopa (Aldomet), fluconazole (Diflucan), itraconazole (Sporanox), erythromycin (Erythrocin, Ilosone, others), phenytoin (Dilantin), lovastatin (Mevacor), pravastatin (Pravachol), simvastatin (Zocor), and many others.

Here's the thing with naltrexone and hemp agrimony: this isn't about one changing how the other works in your bloodstream. It's about your liver. Naltrexone can put some stress on the liver on its own, which is why your doctor may check liver enzymes while you're on it. Hemp agrimony can contain pyrrolizidine alkaloids, natural compounds that are known to be hard on the liver. Stack the two together and you could, in theory, add to that liver strain.

I'll be straight with you: this concern is theoretical, based on what we know about these compounds rather than real cases. Still, since hemp agrimony has little proven benefit and a real liver risk, most experts don't recommend taking it internally at all. Watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please run this by your pharmacist or doctor first.

What Is Naltrexone Used For?

Naltrexone Hydrochloride tablets are used to treat alcohol dependence and to block the effects of opioids in people with opioid dependence. The extended-release injectable suspension — sold as Vivitrol and also available as a generic injectable — is used to treat alcohol dependence and to prevent relapse to opioid dependence after a person has completed opioid detoxification.

All formulations are meant to be part of a broader treatment plan that includes counseling and other support — naltrexone works best when it is not used alone.
