Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hibiscus Sabdariffa interacts with.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP3A4 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP3A4. This interaction has not been reported in humans.

Here's the basic idea: your body uses a liver enzyme called CYP3A4 to clear rilpivirine (Edurant). In a lab dish, hibiscus extract slowed that same enzyme down a bit. If that happened in real life, rilpivirine could build up slightly, which in theory might mean more of its usual side effects, things like headache, trouble sleeping, odd dreams, or mild mood changes.

But please don't lose sleep over this one. This is purely a theoretical, test-tube finding, and it has never actually been reported in people. A cup or two of tart hibiscus tea is very unlikely to matter. Concentrated hibiscus supplements are the bigger question mark. Since staying steady on your HIV medicine matters, just run any regular hibiscus supplement use by your pharmacist or doctor first.

Hibiscus sabdariffa is a tart, cranberry-flavored plant most often consumed as a tea, and its best-studied use is for mildly lowering blood pressure. Evidence for other uses is limited, and while hibiscus tea is generally well tolerated in food amounts, concentrated supplements should be used cautiously and avoided in pregnancy. About Rilpivirine

What Is Rilpivirine Used For?

Brand names: Edurant, Edurant Ped

Rilpivirine tablets (sold as Edurant, Edurant Ped, and generic rilpivirine) are used along with other antiretroviral medicines to treat HIV-1 infection in patients who have not previously been treated with HIV medicines (treatment-naïve). Eligibility depends on age, body weight, and the level of virus in the blood at the start of treatment.

Edurant tablets are also approved, in combination with cabotegravir (Vocabria), for short-term oral use in adults and adolescents who are already virologically suppressed — specifically as a lead-in before starting the long-acting injectable regimen Cabenuva, or as a bridge if a Cabenuva injection dose will be missed.
