Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hibiscus Sabdariffa interacts with.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP3A4 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP3A4. This interaction has not been reported in humans.

Here's the gist for hibiscus and rifampin. Lab studies (done in test tubes, not people) suggest hibiscus extract may gently slow down CYP3A4, one of the enzymes that helps your body process certain medicines. In theory, slowing that enzyme could let a little more rifampin linger in your system.

I'd keep this in perspective, though. This interaction is theoretical and has never actually been reported in humans, so it's considered minor. Rifampin is itself a powerful enzyme booster, so it tends to do the influencing rather than be influenced. A cup of tart hibiscus tea is very unlikely to cause trouble. Still, since rifampin can be tough on the liver, mention any supplements to your pharmacist or doctor so they can keep an eye on things.

Hibiscus sabdariffa is a tart, cranberry-flavored plant most often consumed as a tea, and its best-studied use is for mildly lowering blood pressure. About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
