Dexamethasone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hibiscus Sabdariffa interacts with.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP3A4 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP3A4. This interaction has not been reported in humans.

Here's the gist: your body uses an enzyme called CYP3A4 to clear dexamethasone out of your system. In a lab dish, hibiscus extract slowed that same enzyme down a bit. In theory, that could let dexamethasone hang around a little longer than usual, which might nudge up its steroid effects, things like trouble sleeping, a bigger appetite, mood changes, fluid retention, or higher blood sugar.

The important part: this has only been seen in test tubes, never actually reported in real people. A cup or two of hibiscus tea is very unlikely to cause a problem. Concentrated hibiscus capsules are a bigger unknown, so if you take those daily, just mention it to your pharmacist or doctor.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP2B6 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP2B6. This interaction has not been reported in humans.

Here's the short version: there's a possible link between concentrated hibiscus extract and dexamethasone, but it's pretty weak. In a lab dish, hibiscus appeared to slow down an enzyme called CYP2B6, which is one of the pathways your body uses to clear dexamethasone. In theory, slowing that pathway could let a bit more of the steroid linger, which might mean slightly stronger effects like trouble sleeping, mood changes, higher blood sugar, or fluid retention.

That said, this has never actually been seen in people, and a cup or two of hibiscus tea is very unlikely to cause trouble. The concern, if any, would be with strong supplement doses. Since dexamethasone is not a prodrug, it doesn't flip the direction. If you're using both regularly, just give your pharmacist or doctor a heads-up.

Hibiscus sabdariffa is a tart, cranberry-flavored plant most often consumed as a tea, and its best-studied use is for mildly lowering blood pressure. Evidence for other uses is limited, and while hibiscus tea is generally well tolerated in food amounts, concentrated supplements should be used cautiously and avoided in pregnancy. About Dexamethasone

What Is Dexamethasone Used For?

Brand names: Decadron, Hexadrol, Dexone, Ozurdex, Dexycu, Hemady, Dextenza

Dexamethasone is mainly used to calm inflammation and overactive immune responses. Depending on the product, it treats allergic conditions, skin diseases, adrenal and other hormone disorders, and certain bowel diseases.

Some uses belong only to specific products. Hemady tablets are used with other anti-myeloma medicines for adults with multiple myeloma. Dextenza (an eye insert) treats eye inflammation and pain after eye surgery and eye itching from allergic conjunctivitis. Ozurdex (an eye implant) treats macular edema after retinal vein occlusion, non-infectious uveitis of the back of the eye, and diabetic macular edema. Maxidex and dexamethasone sodium phosphate eye drops treat steroid-responsive inflammation of the front of the eye.
