Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the gist for rilpivirine (Edurant). Your body clears this HIV medicine partly through a liver enzyme called CYP3A4. Lab studies suggest Terminalia arjuna may slow that enzyme down. If that happened in real life, rilpivirine could build up a bit higher than usual, which might mean more of its side effects, such as headache, trouble sleeping, mood changes, or dizziness.

I want to be honest with you, though: this concern comes only from test-tube research, not from studies in people, so it's theoretical at this point. Rilpivirine isn't a prodrug, so a blocker would raise its levels, not weaken it. Because keeping HIV treatment steady really matters, please check with your pharmacist or doctor before adding Terminalia rather than starting or stopping anything on your own.

Terminalia is a group of traditional Ayurvedic tree species (most notably Terminalia arjuna) used for heart, digestive, and general wellness purposes. Some small studies suggest possible benefits for heart and cholesterol health, but the evidence is limited and not strong enough to recommend it as a treatment. About Rilpivirine

What Is Rilpivirine Used For?

Brand names: Edurant, Edurant Ped

Rilpivirine tablets (sold as Edurant, Edurant Ped, and generic rilpivirine) are used along with other antiretroviral medicines to treat HIV-1 infection in patients who have not previously been treated with HIV medicines (treatment-naïve). Eligibility depends on age, body weight, and the level of virus in the blood at the start of treatment.

Edurant tablets are also approved, in combination with cabotegravir (Vocabria), for short-term oral use in adults and adolescents who are already virologically suppressed — specifically as a lead-in before starting the long-acting injectable regimen Cabenuva, or as a bridge if a Cabenuva injection dose will be missed.
