Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the connection to keep in mind. Your HIV medicine rilpivirine (Edurant) is cleared from your body mainly by a liver enzyme called CYP3A4. Phellodendron contains berberine, which lab and early human studies suggest can slow that enzyme down a bit. If that enzyme works slower, rilpivirine could build up in your blood, which may mean more of its side effects, such as headache, trouble sleeping, depression-like mood changes, or in rare cases changes in heart rhythm.

I want to be honest that this is a theoretical concern based on how these compounds behave, not on solid studies of the two together. Still, because keeping your HIV treatment steady really matters, please don't start or stop anything on your own. Check with your pharmacist or doctor first.

Phellodendron is a traditional Chinese medicine bark (Huang Bai) that contains berberine and other plant compounds with antimicrobial and anti-inflammatory activity in lab studies. Human evidence for most uses is limited, and it is often sold in combination with other herbs, making its individual effects hard to judge. About Rilpivirine

What Is Rilpivirine Used For?

Brand names: Edurant, Edurant Ped

Rilpivirine tablets (sold as Edurant, Edurant Ped, and generic rilpivirine) are used along with other antiretroviral medicines to treat HIV-1 infection in patients who have not previously been treated with HIV medicines (treatment-naïve). Eligibility depends on age, body weight, and the level of virus in the blood at the start of treatment.

Edurant tablets are also approved, in combination with cabotegravir (Vocabria), for short-term oral use in adults and adolescents who are already virologically suppressed — specifically as a lead-in before starting the long-acting injectable regimen Cabenuva, or as a bridge if a Cabenuva injection dose will be missed.
