Riociguat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Vitamin E interacts with.

Theoretically, vitamin E might induce metabolism of CYP3A4, possibly reducing the levels CYP3A4 substrates.
Vitamin E appears to bind with the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of vitamin E and other drugs affected by these enzymes.

Here's the gist for Riociguat (Adempas): your body clears it using a liver enzyme called CYP3A4. In lab-based theory, high-dose vitamin E may gently nudge that enzyme to work a little faster. If that happened, your body could break down riociguat a bit quicker, which might leave less of it in your system and make it slightly less effective at relaxing the lung blood vessels and easing your breathing or exercise tolerance.

I want to be honest with you though: this is theoretical, based only on how things look on paper, not real patient cases. The amount of vitamin E in food won't matter here. If you're thinking about high-dose vitamin E supplements, just run it past your pharmacist or doctor first so nobody changes your riociguat without a plan.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold under the brand names Adempas and Riociguat — are approved for adults with two types of high blood pressure in the lungs. The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots that have blocked or narrowed the arteries in the lungs. Riociguat is used when surgery has not fully fixed the problem, or when surgery is not an option.

The second is pulmonary arterial hypertension (PAH), a progressive disease where the arteries in the lungs become stiff and narrowed. In PAH, riociguat helps improve exercise ability, functional class (how well you can get around day-to-day), and slows disease worsening. It can be used alone or combined with other PAH medicines called endothelin receptor antagonists or prostanoids.
