Vandetanib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Vitamin E interacts with.

Theoretically, vitamin E might induce metabolism of CYP3A4, possibly reducing the levels CYP3A4 substrates.
Vitamin E appears to bind with the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of vitamin E and other drugs affected by these enzymes.

Here's the gist for Vandetanib (Caprelsa): your body clears this cancer drug partly through a liver enzyme called CYP3A4. There's a theory that high-dose vitamin E nudges that enzyme to work a little faster. If that happened, your body could clear vandetanib more quickly, which might lower its blood levels and, in theory, make it less effective at controlling your thyroid cancer.

I want to be honest, though: this is based on lab pharmacology, not real patient studies, so we don't actually know if it matters in practice. Everyday food amounts of vitamin E are very unlikely to cause trouble. The concern, if any, is concentrated high-dose supplements. Because vandetanib is serious cancer therapy, please don't start or stop any supplement without checking with your oncology team or pharmacist first.

Caprelsa (vandetanib) is used to treat medullary thyroid cancer — a type of thyroid cancer — in adults whose tumor cannot be surgically removed or has spread (locally advanced or metastatic disease) and is causing symptoms or actively growing.

Caprelsa should be used with extra caution in patients whose cancer is slow-growing and not causing symptoms, because the serious risks of the medicine must be carefully weighed against any potential benefit in that setting.
