Vandetanib falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Here's the concern with this pairing. Vandetanib (Caprelsa) is already known to stretch out the heart's electrical recovery time, what we call the QT interval. The drug even carries a serious warning about this. Ephedra, thanks to its stimulant compounds (ephedrine), can do the same thing on its own, and EKG changes have shown up even at normal doses in healthy people.

Stack the two together and the effects can add up, raising the risk of a dangerous, irregular heart rhythm. That can feel like a racing or pounding heartbeat, dizziness, fainting, or in rare cases something life-threatening. This isn't a small worry given how powerful both are. Ephedra is also banned in U.S. supplements. Please don't combine them, and talk with your oncologist or pharmacist about safer options.

Brand name: Caprelsa

Caprelsa (vandetanib) is used to treat medullary thyroid cancer — a type of thyroid cancer — in adults whose tumor cannot be surgically removed or has spread (locally advanced or metastatic disease) and is causing symptoms or actively growing.

Caprelsa should be used with extra caution in patients whose cancer is slow-growing and not causing symptoms, because the serious risks of the medicine must be carefully weighed against any potential benefit in that setting.
