Caffeine (prescription drug) falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Wheatgrass interacts with.
In vitro research shows that wheatgrass induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for prescription caffeine and wheatgrass. Your body clears caffeine mainly through a liver enzyme called CYP1A2. Lab studies suggest wheatgrass may speed up that enzyme, which in theory means caffeine could get broken down a little faster and not stick around as long. The practical result would be a slightly weaker or shorter caffeine effect, so you might notice less of the alertness or stimulation you're taking it for.
I want to be honest about how thin this evidence is. It comes only from test-tube research, not real people, so the real-world impact is probably small. Wheatgrass also doesn't reduce or block caffeine's effects in a dangerous way. If you rely on prescription caffeine for a specific reason, just mention the wheatgrass to your pharmacist or doctor and see what they think.

Caffeine citrate injection and oral solution are used for the short-term treatment of apnea of prematurity. This means pauses in breathing in premature infants. The label lists the age range as infants between 28 weeks and a later gestational age, and that range is cut off in the text provided.
Over-the-counter caffeine tablets, such as Diurex and the Stay Awake products, are taken by mouth by adults and older children. Their labels point to fatigue and drowsiness. Diurex directions also mention taking it while symptoms persist.
