Wheatgrass and Bendamustine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Bendamustine

Bendamustine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Wheatgrass interacts with.

Theoretically, wheatgrass might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that wheatgrass induces CYP1A2 enzymes.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic idea with wheatgrass and Bendamustine. Your body partly clears Bendamustine using a liver enzyme called CYP1A2. Lab studies suggest wheatgrass may speed up (induce) that enzyme, which in theory could clear the drug a little faster and slightly soften how well it works. Bendamustine is not a prodrug, so faster clearance would lean toward less effect, not more.

I want to be honest about the strength here: this is only a theoretical concern based on test-tube research, not real patient studies, so it may not matter in practice. Still, because this is serious cancer therapy where steady dosing counts, please don't start wheatgrass on your own. Run it by your oncology team or pharmacist first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

From our pharmacist’s overview on the Bendamustine page

“Bendamustine (Treanda, Belrapzo, Vivimusta) is IV chemotherapy for chronic lymphocytic leukemia and certain lymphomas, and the liver clears it through an enzyme called CYP1A2. That enzyme is why nearly all 87 supplements on this page appear here, since about 80 of them can push CYP1A2 up or down, which could either raise side effects o…”

About Wheatgrass

What is Wheatgrass?

Wheatgrass
  • General nutrition support
  • Digestive health
  • Antioxidant support
  • Energy and wellness
  • Arthritis symptoms
  • Detox or cleansing routines

Wheatgrass is the young grass of the wheat plant, taken as a juice or powder, and is mainly used as a concentrated source of vitamins and plant nutrients. Solid scientific evidence for most health claims is limited, and it is best viewed as a nutritional add-on rather than a treatment. It is generally well tolerated, but people with celiac disease, wheat allergy, or who are pregnant should be cautious and check with a healthcare provider.
