Wheatgrass and Fenfluamine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Fenfluamine

Fenfluamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Wheatgrass interacts with.

Theoretically, wheatgrass might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that wheatgrass induces CYP1A2 enzymes.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Fintepla (fenfluramine) and wheatgrass. Your body partly clears fenfluramine using a liver enzyme called CYP1A2. Some early lab research suggests wheatgrass might speed that enzyme up. If it does, your body could break fenfluramine down a little faster, which in theory means slightly lower levels and possibly weaker seizure control.

I want to be honest about how soft this evidence is. It comes only from test-tube studies, not from people, so we really don't know if it matters in real life. Since fenfluramine is a seizure medicine where steady levels count, I'd still mention it to your pharmacist or neurologist before adding wheatgrass, and never adjust your dose on your own.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Fenfluamine page

“Alcohol is the danger I would flag first here. Fenfluamine (Fintepla) treats seizures in severe childhood epilepsies such as Dravet syndrome, and it is sedating by itself, so beer, wine, kava, and the other sedating ingredients on this page, 78 in all, can pile extra drowsiness on top, at worst to the point of suppressed breathing.”

About Wheatgrass

What is Wheatgrass?

Wheatgrass
  • General nutrition support
  • Digestive health
  • Antioxidant support
  • Energy and wellness
  • Arthritis symptoms
  • Detox or cleansing routines

Wheatgrass is the young grass of the wheat plant, taken as a juice or powder, and is mainly used as a concentrated source of vitamins and plant nutrients. Solid scientific evidence for most health claims is limited, and it is best viewed as a nutritional add-on rather than a treatment. It is generally well tolerated, but people with celiac disease, wheat allergy, or who are pregnant should be cautious and check with a healthcare provider.

About Fenfluramine

What Is Fenfluramine Used For?

Brand name: Fintepla

Fintepla (fenfluramine oral solution) is approved to treat seizures associated with Dravet syndrome (DS) and Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older. It is used alongside other antiseizure medications.

Both conditions are severe, often hard-to-control forms of epilepsy that typically begin in childhood. Fintepla is available only through a restricted program called the Fintepla REMS because of serious heart-related risks.
