Pirfenidone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Wheatgrass interacts with.

Theoretically, wheatgrass might decrease the levels and clinical effects of CYP1A2 substrates.

Here's the gist for Esbriet (pirfenidone): your body relies on a liver enzyme called CYP1A2 to break this medicine down. Lab tests suggest wheatgrass may rev that enzyme up a bit. If it does, your body could clear pirfenidone a little faster, which in theory might leave you with lower levels and slightly less of the lung-protecting benefit you're counting on for pulmonary fibrosis.

I want to be honest about how soft this is. The concern comes only from test-tube research, not from people, so it's more of a theoretical caution than a proven problem. Pirfenidone isn't a prodrug, so the worry here is reduced effect rather than more side effects. Still, before adding wheatgrass, run it by your pharmacist or doctor so they can keep an eye on things.

Esbriet capsules and tablets, pirfenidone capsules, and pirfenidone tablets are each specifically approved for IPF. No other uses are supported by the available product labels.
