Macitentan, Tadalafil falls under ANTIHYPERTENSIVE DRUGS — the group White Horehound interacts with.

Evidence from animal research suggests that taking white horehound extract lowers blood pressure. Theoretically, taking white horehound with antihypertensive drugs might enhance therapeutic effects and increase the risk of hypotension. Some antihypertensive drugs include captopril (Capoten), enalapril (Vasotec), losartan (Cozaar), valsartan (Diovan), diltiazem (Cardizem), Amlodipine (Norvasc), hydrochlorothiazide (HydroDiuril), furosemide (Lasix), and many others.

Opsynvi combines two medicines (macitentan and tadalafil) that both work to relax blood vessels and lower pressure in the lungs, and they can lower your overall blood pressure too. White horehound has shown a mild blood-pressure-lowering effect, but only in animal studies, so this is a theoretical concern rather than something clearly seen in people.

The idea is that stacking the herb on top of Opsynvi might add to the drop in blood pressure. If your pressure dips too low, you could feel dizzy, lightheaded, or faint, especially when standing up quickly. Occasional horehound cough drops are unlikely to matter much. Concentrated extracts or supplements are the bigger question mark, so please check with your pharmacist or doctor before adding them.
