Alprazolam falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the short version: your body relies on a liver enzyme called CYP3A4 to break down alprazolam (Xanax). Piperine, the spicy compound in white pepper, has shown in lab dishes that it can slow this enzyme down. If that happened in a person, the alprazolam would clear out a little slower and could build up, meaning you might feel more drowsy, dizzy, foggy, or unsteady than usual.

Now the reassuring part: this is only a theoretical concern based on test-tube research, not real patient cases. The sprinkle of white pepper on your dinner is very unlikely to cause any trouble. The bigger question would be concentrated piperine or pepper supplements. If you're considering those, just run it by your pharmacist or doctor first.

What Is Alprazolam Used For?

Brand names: Xanax, Niravam

Alprazolam treats generalized anxiety disorder (GAD) and panic disorder, with or without agoraphobia, in adults. These uses are supported across multiple alprazolam products, including tablets, orally disintegrating tablets, oral solution, and extended-release tablets.

Xanax (tablet) is indicated for the acute treatment of GAD and for panic disorder. Xanax XR (extended-release tablet) and alprazolam extended-release tablets are indicated specifically for panic disorder with or without agoraphobia — not for GAD. Alprazolam orally disintegrating tablets and alprazolam oral solution are indicated for both GAD and panic disorder.
