Aspirin, Aluminum Hydroxide, Codeine Phosphate, Magnesium Hydroxide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that some constituents of white pepper inhibit cytochrome P450 2D6 (CYP2D6). Theoretically, concomitant use may affect drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the key thing about this combo: the codeine in your medication is a prodrug. Your body has to convert it into morphine using an enzyme called CYP2D6 before it can ease your pain. Lab studies hint that piperine, the spicy compound in white pepper, might block that same enzyme a little.

If that happened, less codeine would turn into morphine, so you might get weaker pain relief than expected. The good news is this is theoretical, based only on test-tube work, and the small sprinkle of pepper you put on food almost certainly won't matter. The aspirin and antacid parts aren't affected here. If you ever take concentrated piperine supplements, just check with your pharmacist or doctor first.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits platelet aggregation. Theoretically, white pepper might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
Some anticoagulant/antiplatelet drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Your combination product contains aspirin, which already thins the blood by keeping platelets from clumping together. The concern here is that piperine (the spicy compound in white pepper) appears to do something similar in lab dishes. Stacked on top of aspirin, the theory is that bleeding risk could tick up a bit, showing up as easier bruising, nosebleeds, or a cut that takes longer to stop.

That said, this is based only on test-tube research, not real people, so it's far from proven. Sprinkling white pepper on your dinner almost certainly won't matter. Concentrated piperine supplements are a different story. Before adding one, run it by your pharmacist or doctor, especially since you're already on aspirin.
