Aspirin, Caffeine, Dihydrocodeine Bitartrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that some constituents of white pepper inhibit cytochrome P450 2D6 (CYP2D6). Theoretically, concomitant use may affect drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the wrinkle with Synalgos DC: the dihydrocodeine part is a prodrug. Your body uses an enzyme called CYP2D6 to turn it into the active form that actually relieves pain. White pepper's piperine has shown a mild ability to block that same enzyme in lab studies. So in theory, less of the dihydrocodeine would get switched on, and you might notice weaker pain relief rather than more side effects.

The good news is this is only theoretical, based on test-tube work, and everyday seasoning amounts of pepper are very unlikely to matter. Concentrated piperine supplements are the bigger question mark. If you're considering one, just run it by your pharmacist or doctor first so your pain control stays on track.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist with Synalgos DC and white pepper. The piperine in pepper has shown, in lab dishes only, that it can slow down an enzyme called CYP3A4. That same enzyme helps clear part of this combination pill, including the dihydrocodeine portion. In theory, slowing that enzyme could let the medicine linger a bit longer, which might bump up effects like drowsiness, dizziness, constipation, or slowed breathing from the opioid piece, plus jitteriness from the caffeine.

That said, this is theoretical and based on test-tube data, not real people. Sprinkling white pepper on your food almost certainly won't matter. Concentrated piperine supplements are a different story, so check with your pharmacist or doctor before using those alongside this medication.

ANTICOAGULANT/ANTIPLATELET DRUGS

Aspirin, Caffeine, Dihydrocodeine Bitartrate also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits platelet aggregation. Theoretically, white pepper might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
Some anticoagulant/antiplatelet drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the gist with Synalgos DC, which combines aspirin, caffeine, and dihydrocodeine. The piece that matters here is the aspirin, since it already thins your blood a bit by keeping platelets from clumping. White pepper contains piperine, and in lab dish studies piperine seemed to slow platelet clumping too. So in theory, stacking the two could nudge your bleeding risk a little higher (think easier bruising, nosebleeds, or a cut that's slow to stop).

I want to be honest though: this is only test-tube evidence, not proof in people, and the pepper you sprinkle on dinner is very unlikely to cause trouble. Concentrated piperine supplements are the bigger question mark. If you're considering one, just run it by your pharmacist or doctor first.
