Bivalirudin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group White Pepper interacts with.
Some anticoagulant/antiplatelet drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: bivalirudin (Angiomax) is a powerful blood thinner given through an IV, usually during heart procedures like angioplasty or stent placement. It works by directly blocking thrombin, a key protein in clotting. White pepper's active compound, piperine, has shown in lab dishes that it can mildly slow platelets from clumping together.
Put those two together and the theoretical concern is a slightly higher bleeding risk. Honestly though, this is based only on test-tube research, not real people, and bivalirudin is given in a closely monitored hospital setting where your care team watches you carefully. Normal seasoning amounts of pepper are very unlikely to matter. If you take concentrated piperine supplements, just mention it to your doctor beforehand.
