Iloprost falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits platelet aggregation. Theoretically, white pepper might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
Some anticoagulant/antiplatelet drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here is the connection worth knowing about. Besides relaxing the blood vessels in your lungs, iloprost (Ventavis) also makes your platelets a little less sticky, which can gently raise bleeding risk on its own. White pepper's main compound, piperine, has shown a similar platelet-slowing effect in lab studies. Stack the two and, in theory, you could bruise more easily or bleed a bit longer than usual.

The key word is theory. This is based on test-tube research, not real patients, and the everyday sprinkle of pepper on your food is very unlikely to cause a problem. Concentrated piperine supplements are where I'd pause. Before starting any pepper or piperine capsule, check with your pharmacist or doctor, and watch for unusual bruising, nosebleeds, or bleeding gums.
