Ledipasvir, Sofosbuvir falls under P-GLYCOPROTEIN SUBSTRATES — the group White Pepper interacts with.

Theoretically, white pepper might increase levels of p-glycoprotein substrates. The piperine constituent of white pepper seems to inhibit p-glycoprotein in vitro.
Some drugs metabolized by p-glycoprotein include some chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the gist for Harvoni (ledipasvir/sofosbuvir). Both of these ingredients are carried around your body by a transporter called p-glycoprotein, which acts like a gatekeeper for how much drug gets absorbed. The piperine in white pepper has shown, in test-tube studies, that it can block that gatekeeper. In theory, that could let a bit more of your hepatitis C medicine into your system, possibly nudging up side effects like tiredness, headache, or nausea.

The good news: this is only a theoretical concern based on lab work, not real patient cases. Everyday cooking with white pepper is very unlikely to cause trouble. Concentrated piperine supplements are a different story, so it's worth a quick chat with your pharmacist or doctor before taking those during treatment.

What Is Ledipasvir and Sofosbuvir Used For?

Brand name: Harvoni

Ledipasvir and sofosbuvir treats chronic hepatitis C virus (HCV) infection in adults and pediatric patients 3 years of age and older. It is approved for HCV genotypes 1, 4, 5, and 6 in patients without cirrhosis or with compensated cirrhosis (mild-to-moderate liver scarring that the liver is still managing), and for genotype 1 with decompensated cirrhosis (advanced liver disease) when used together with ribavirin.

It is also approved for liver transplant recipients with HCV genotype 1 or 4 infection who do not have cirrhosis or have compensated cirrhosis, in combination with ribavirin. Both Harvoni and the generic ledipasvir and sofosbuvir tablets share these indications.
