Selumetinib Sulfate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist for Koselugo (selumetinib): your child's body clears this medicine partly through a liver enzyme called CYP3A4. In lab studies, piperine (the spicy compound in white pepper) seems to slow that enzyme down a bit. If the enzyme works slower, selumetinib could clear out more slowly and build up, which in theory means stronger effects and more side effects, things like nausea, diarrhea, mouth sores, skin rash, or fatigue.

I want to be honest about how soft this evidence is. It comes only from test-tube work, not real patients, and normal cooking amounts of white pepper are very unlikely to matter. Concentrated piperine supplements are the bigger question. Since selumetinib is a serious cancer medicine, please check with your pharmacist or oncology team before adding any pepper-based supplement.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits platelet aggregation. Theoretically, white pepper might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
Some anticoagulant/antiplatelet drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the thing to keep in mind with Koselugo (selumetinib) and white pepper. The concern on file is about bleeding. In lab studies, piperine (the spicy compound in pepper) seems to make platelets a little less sticky, so in theory it could add to the bleeding risk of blood thinners. Selumetinib itself isn't a true anticoagulant, but it can lower platelet counts and cause bruising or bleeding in some people, so the worry is that the two together could nudge that risk a bit higher.

The honest part: this is theoretical and based only on test-tube and animal data, not real patients. Normal kitchen amounts of white pepper on your food are very unlikely to cause a problem. Concentrated piperine supplements are a different story. If your child notices easy bruising, nosebleeds, or bleeding gums, check in with your care team or pharmacist.

What Is Selumetinib Used For?

Brand name: Koselugo

Koselugo (selumetinib) is approved for adults and children 1 year of age and older who have neurofibromatosis type 1 (NF1) — a genetic condition that causes tumors to grow on nerve tissue — when they also have symptomatic, inoperable plexiform neurofibromas (large, deep nerve tumors that cannot be surgically removed and are causing problems).

Both the Koselugo capsule and Koselugo oral granule formulations are approved for this use. The oral granule form is intended for patients who have difficulty swallowing whole capsules.
