Aspirin, Butalbital, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Wild Cherry interacts with.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
If you take Fiorinal (a combination of aspirin, butalbital, and caffeine), here's the gist with wild cherry. Lab studies hint that wild cherry might slow down an enzyme called CYP3A4, which your liver uses to clear certain medicines. The butalbital piece (a barbiturate) is partly handled by this enzyme, so in theory it could build up a little and leave you feeling more drowsy, foggy, or sluggish than usual.
Here's the honest part: this is based only on test-tube research, and it has never actually been seen in real patients. So I wouldn't lose sleep over it. Normal cough-syrup amounts are unlikely to cause trouble. Still, if you plan to use concentrated wild cherry regularly, run it by your pharmacist or doctor first.
