What’s inside
Full disclosure
This product contains one active ingredient: acetyl-L-carnitine, a compound your body naturally makes from the amino acid carnitine. It plays a role in how your cells produce energy and may have effects on brain chemistry, particularly on serotonin levels.
The capsule itself is made from gelatin, with magnesium stearate added as a flow agent.
Does it work?
Moderate evidence
Acetyl-L-carnitine is rated possibly effective for age-related cognitive decline, Alzheimer disease, alcohol use disorder, diabetic neuropathy (nerve damage from diabetes), and depression. These ratings mean the evidence is suggestive but not yet definitive — we have some research pointing in the right direction, but we need more solid proof before we can say for certain it works for these uses.
How safe is it?
Well-documented data
Acetyl-L-carnitine is generally well tolerated in the short term, though long-term safety hasn't been fully studied. The most common side effects are agitation, dry mouth, headache, insomnia, and reduced appetite.
Some people report a fishy odor in urine, breath, or sweat — that's a normal metabolite of the supplement. Nausea, vomiting, and digestive upset can occur but don't usually happen more often than with placebo.
In people with Alzheimer disease, psychiatric changes like depression, mania, confusion, and aggression have been noted, though it's unclear whether the supplement caused them or the condition itself did. There isn't enough reliable safety information during pregnancy, so it's best avoided unless your doctor advises otherwise.
Safety while breastfeeding is unknown, so avoiding it is the safer choice.
Meds to double-check
Moderate interaction found
Before you start this product, double-check with your pharmacist if you take serotonergic drugs like antidepressants (risk of serotonin syndrome), blood thinners such as warfarin or acenocoumarol (bleeding risk), or thyroid hormone replacement (reduced effectiveness). These are Moderate-severity interactions.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Acetyl-L-carnitine may be worth considering if you're looking into support for cognitive decline, Alzheimer disease, or diabetic nerve pain — the evidence is promising but not airtight. But if you take serotonergic antidepressants, blood thinners, or thyroid hormone, you need to check with your pharmacist first.

Acetyl L-Carnitine by Core Nutritionals Drug Interactions
HelloPharmacist Interaction Report
Acetyl L-Carnitine by Core Nutritionals interacts with several medication types.
The most serious concern is with serotonergic drugs (antidepressants, some pain relievers, migraine medications) — acetyl-L-carnitine may increase brain serotonin levels and theoretically raise the risk of serotonin syndrome, a potentially serious condition marked by agitation, confusion, rapid heart rate, and high blood pressure.
Read the full breakdown — every affected drug type, severity by severity
You'll also want to check if you take blood thinners. Acetyl-L-carnitine may increase how well warfarin and acenocoumarol (shorter-acting blood thinners similar to warfarin) work, which could raise your bleeding risk.
Additionally, if you take thyroid hormone replacement, this supplement might reduce its effectiveness by blocking how your cells absorb the hormone.
Altogether, these interactions span 203 individual medications. Use the medication checker below to look up your exact prescriptions before starting this product.

Acetyl-L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine, the parent compound of acetyl-L-carnitine, might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant that is similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation when L-carnitine was taken with acenocoumarol. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product. It is unclear if such an interaction would also occur with acetyl-L-carnitine.
Serotonergic Drugs
Theoretically, acetyl-L-carnitine might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders, when taken with serotonergic drugs.
Animal research shows that acetyl-L-carnitine can increase levels of serotonin in the brain.
Thyroid Hormone
Theoretically, acetyl-L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism. It is unclear if such an interaction would occur with acetyl-L-carnitine.
Warfarin (Coumadin)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine, the parent compound of acetyl-L-carnitine, might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with acetyl-L-carnitine and warfarin.
