What’s inside
Full disclosure
This capsule contains one active ingredient: 450 mg of apple cider vinegar powder. The remaining ingredients—maltodextrin, hypromellose, stearic acid, and silicon dioxide—are inactive fillers and capsule materials used to form and preserve the supplement.
Does it work?
Couldn't assess
The evidence on apple cider vinegar for the purposes listed—acne, athletic performance, cognitive function, dental plaque, depression, and atopic dermatitis (eczema)—is insufficient to establish a reliable rating. That means we don't yet have solid data to say whether it works for these uses.
If you're considering this supplement for a specific condition, talk it over with your doctor or pharmacist to see what the best option is for you.
How safe is it?
Well-documented data
In food amounts, apple cider vinegar is generally well tolerated. When used orally for short-term medicinal purposes, it also appears to be well tolerated.
However, the safety data for large supplement doses and long-term use is limited, and long-term use in larger amounts may be unsafe. Rare serious adverse effects from long-term oral use have been reported, including low potassium (hypokalemia), elevated kidney hormones, and bone loss (osteoporosis)—effects seen in one person who took 250 mL daily for 6 years.
Topical use (applying it to skin) carries a risk of chemical burns and skin irritation. There is no safety data on file for pregnancy and breastfeeding, so talk with your doctor or pharmacist if you're pregnant, planning to become pregnant, or breastfeeding.
Meds to double-check
Moderate interaction found
Before you take this supplement, double-check with your doctor or pharmacist if you're on digoxin (Lanoxin), any diuretic or water pill, insulin, or any antidiabetes medication. All of these carry a documented risk of interaction with apple cider vinegar.
The bottom line
Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Apple cider vinegar in food amounts is well tolerated, but supplement doses come with limited long-term safety data and potentially serious interactions—especially with heart, blood pressure, and diabetes medications. If you take digoxin, diuretics, insulin, or other antidiabetes drugs, check with your doctor or pharmacist before starting this product.
For everyone else, discuss it with your healthcare provider to make sure it's right for you.

Apple Cider Vinegar 450 mg by NOW Drug Interactions
HelloPharmacist Interaction Report
NOW's Apple Cider Vinegar 450 mg contains one active ingredient—apple cider vinegar powder—that has documented interactions with several medication types.
The most serious concern is with digoxin (Lanoxin), a heart medication: apple cider vinegar may lower your potassium levels, and low potassium can increase the risk of digoxin toxicity and heart problems.
Read the full breakdown — every affected drug type, severity by severity
Diuretic drugs (water pills) also carry a Moderate risk. These medications already remove potassium from your body, and apple cider vinegar may worsen that effect, leading to dangerously low potassium (hypokalemia).
Insulin and antidiabetes drugs are also affected—insulin at Moderate severity because of the same potassium-lowering concern, and antidiabetes drugs at Minor severity because apple cider vinegar might lower blood sugar and add to the effect of your diabetes medication, raising the risk of low blood sugar (hypoglycemia).
Altogether, these interactions span 162 individual medications. Before you start this supplement, run your exact medications through the checker on this page or discuss them with your doctor or pharmacist—especially if you take any heart, blood pressure, diabetes, or kidney medications.

Digoxin (Lanoxin)
Theoretically, concomitant use of apple cider vinegar with digoxin might increase the risk of cardiac toxicity.
A case of hypokalemia related to chronic use of apple cider vinegar has been reported. Theoretically, overuse of apple cider vinegar could decrease potassium levels, increasing the risk of toxicity with digoxin.
Diuretic Drugs
Theoretically, concomitant use of apple cider vinegar with diuretic drugs might increase the risk of hypokalemia.
A case of hypokalemia related to chronic use of apple cider vinegar has been reported. There is some concern that people taking apple cider vinegar along with potassium depleting diuretics might have an increased risk for hypokalemia.
Insulin
Theoretically, concomitant use of apple cider vinegar with insulin might increase the risk of hypokalemia.
A case of hypokalemia related to chronic use of apple cider vinegar has been reported. Theoretically, overuse of apple cider vinegar concomitantly with insulin might increase the risk of hypokalemia.
Antidiabetes Drugs
Theoretically, taking apple cider vinegar with antidiabetes drugs might increase the risk of hypoglycemia.
Apple cider vinegar might reduce fasting and postprandial blood glucose levels and decrease gastric emptying in people with diabetes. However, not all research agrees. Theoretically, it might have additive effects on glucose levels when used with antidiabetes drugs.
