Hydrochlorothiazide, Lisinopril falls under DIURETIC DRUGS — the group Apple Cider Vinegar interacts with.

Theoretically, concomitant use of apple cider vinegar with diuretic drugs might increase the risk of hypokalemia.
A case of hypokalemia related to chronic use of apple cider vinegar has been reported. There is some concern that people taking apple cider vinegar along with potassium depleting diuretics might have an increased risk for hypokalemia.

Here's the gist of it: your blood pressure pill is a combo. The hydrochlorothiazide (the 'water pill' half) can lower your body's potassium. Apple cider vinegar may add to that effect, since there's a reported case of someone developing low potassium after taking a lot of it over a long time. The lisinopril half actually tends to raise potassium, so for many folks these may balance out, but I wouldn't count on that.

If potassium dips too low, you might feel tired, crampy, weak, or notice an irregular heartbeat. Honestly, the evidence here is thin and based mostly on one case, so a splash of vinegar on a salad is unlikely to be a problem. Concentrated daily doses are the bigger concern. Please check in with your pharmacist or doctor before making it a habit.

What Is Lisinopril and Hydrochlorothiazide Used For?

Brand names: Zestoretic, Prinzide

Lisinopril and hydrochlorothiazide tablets — sold as the brand Zestoretic and as the generic combination — are used to treat high blood pressure (hypertension). Lowering blood pressure reduces the risk of serious cardiovascular events such as stroke and heart attack.

This is a fixed-dose combination product and is not intended as a first-line (initial) treatment for high blood pressure. It is typically prescribed for patients whose blood pressure is not adequately controlled on a single medication alone.
