Apple Cider Vinegar and Tolazamide

Here is the interaction we found between these two, with the evidence behind it.

Minor

ANTIDIABETES DRUGS

Why this applies to Tolazamide

Tolazamide falls under ANTIDIABETES DRUGS — the group Apple Cider Vinegar interacts with.

Theoretically, taking apple cider vinegar with antidiabetes drugs might increase the risk of hypoglycemia.
Apple cider vinegar might reduce fasting and postprandial blood glucose levels and decrease gastric emptying in people with diabetes. However, not all research agrees. Theoretically, it might have additive effects on glucose levels when used with antidiabetes drugs.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: tolazamide already lowers your blood sugar by pushing your pancreas to release more insulin. Apple cider vinegar may nudge blood sugar down a little too, partly by slowing how fast your stomach empties. Stack the two together and, in theory, your sugar could drop a bit lower than expected. That can show up as shakiness, sweating, dizziness, hunger, or feeling foggy, the usual signs of low blood sugar (hypoglycemia).

That said, this is a minor, mostly theoretical concern, and the research on vinegar is mixed and not very strong. A splash on a salad is unlikely to cause trouble. If you take daily ACV shots or capsules, just keep an eye on your readings and chat with your pharmacist or doctor before changing anything.

From our pharmacist’s overview on the Tolazamide page

“Tolazamide (Tolinase) lowers blood sugar in type 2 diabetes, and I would handle its supplement list with care, because nearly everything listed can drag blood sugar lower still. Herbs and foods with glucose-lowering effects dominate the file, and stacked on a pill that already lowers glucose they raise the odds of hypoglycemia, blood s…”

About Apple Cider Vinegar

What is Apple Cider Vinegar?

Apple Cider Vinegar
  • Blood sugar support
  • Weight management
  • Digestive upset
  • Appetite control
  • Skin and hair care

Apple cider vinegar is a fermented apple product that many people use hoping to help with blood sugar, weight, or digestion. The evidence for most of these uses is limited and mixed, and the modest effects seen in small studies are not a substitute for proven treatments. When diluted and used in small amounts it is generally safe for most people, but undiluted vinegar can harm teeth and the throat.

About Tolazamide

What Is Tolazamide Used For?

Brand name: Tolinase

  • Tolazamide is used along with diet and exercise, and sometimes with other medications, to treat type 2 diabetes (condition in which the body does not use insulin normally and, therefore, cannot control the amount of sugar in the blood). Tolazamide is in a class of medications called sulfonylureas. Tolazamide lowers blood sugar by causing the pancreas to produce insulin (a natural substance that is needed to break down sugar in the body) and helping the body use insulin efficiently. This medication will only help lower blood sugar in people whose bodies produce insulin naturally. Tolazamide is not used to treat type 1 diabetes (condition in which the body does not produce insulin and, therefore, cannot control the amount of sugar in the blood) or diabetic ketoacidosis (a serious condition that may occur if high blood sugar is not treated).

  • Over time, people who have diabetes and high blood sugar can develop serious or life-threatening complications, including heart disease, stroke, kidney problems, nerve damage, and eye problems. Taking medication(s), making lifestyle changes (e.g., diet, exercise, quitting smoking), and regularly checking your blood sugar may help to manage your diabetes and improve your health. This therapy may also decrease your chances of having a heart attack, stroke, or other diabetes-related complications such as kidney failure, nerve damage (numb, cold legs or feet; decreased sexual ability in men and women), eye problems, including changes or loss of vision, or gum disease. Your doctor and other healthcare providers will talk to you about the best way to manage your diabetes.
