Pramlintide falls under ANTIDIABETES DRUGS — the group Apple Cider Vinegar interacts with.

Theoretically, taking apple cider vinegar with antidiabetes drugs might increase the risk of hypoglycemia.
Apple cider vinegar might reduce fasting and postprandial blood glucose levels and decrease gastric emptying in people with diabetes. However, not all research agrees. Theoretically, it might have additive effects on glucose levels when used with antidiabetes drugs.

Here's the gist: both apple cider vinegar and Symlin (pramlintide) can lower blood sugar, and interestingly they work in a similar way. Pramlintide slows how fast food leaves your stomach so sugar rises more gently after meals, and acetic acid in vinegar may do something similar. Stack them on top of your mealtime insulin and the effects could add up, nudging your blood sugar lower than expected. That raises the chance of a low (hypoglycemia), which can show up as shakiness, sweating, dizziness, or confusion.

That said, this is rated minor and partly theoretical, and the vinegar evidence is mixed. A splash on a salad is unlikely to cause trouble. Concentrated ACV doses are where I'd be more cautious. Keep checking your sugars and chat with your doctor or pharmacist before adding ACV supplements.

What Is Pramlintide Injection Used For?

Brand name: Symlin

SymlinPen (pramlintide) is used as an add-on treatment for adults with type 1 or type 2 diabetes who already use mealtime insulin but still haven't reached their target blood sugar levels — even with optimal insulin therapy.

It is not a replacement for insulin. It works alongside insulin, given right before major meals, to reduce the spike in blood sugar that happens after eating.
