A case of hypokalemia related to chronic use of apple cider vinegar has been reported. There is some concern that people taking apple cider vinegar along with potassium depleting diuretics might have an increased risk for hypokalemia.

Here's a twist worth knowing. The concern on file is that apple cider vinegar plus a diuretic could lower potassium too much. But spironolactone is actually a potassium-sparing diuretic, meaning it holds onto potassium rather than flushing it out. So the usual worry about vinegar dropping your potassium doesn't line up neatly with this particular water pill the way it might with something like furosemide.

The evidence here is thin anyway (mostly a single case report and theory), so I wouldn't lose sleep over a splash of vinegar in your salad. If you take large daily doses of concentrated ACV, just mention it to your pharmacist, since spironolactone already affects potassium and your levels are something to keep an eye on.

About Spironolactone

What Is Spironolactone Used For?

Brand names: Aldactone, Spiroton, Carospir

Spironolactone tablets (including Aldactone) are used to treat moderate-to-severe heart failure, high blood pressure, and fluid retention (edema) caused by liver cirrhosis or nephrotic syndrome. They are also used to diagnose and manage primary hyperaldosteronism — a condition where the adrenal glands produce too much of the hormone aldosterone.

Spironolactone oral suspension shares several of these uses — heart failure, high blood pressure, and edema from liver cirrhosis — but is not the same as Aldactone and is not therapeutically interchangeable with it. Your prescriber will choose the right form for your situation.
