Amiloride, Hydrochlorothiazide falls under DIURETIC DRUGS — the group Apple Cider Vinegar interacts with.

Theoretically, concomitant use of apple cider vinegar with diuretic drugs might increase the risk of hypokalemia.
A case of hypokalemia related to chronic use of apple cider vinegar has been reported. There is some concern that people taking apple cider vinegar along with potassium depleting diuretics might have an increased risk for hypokalemia.

Here's the nice thing about your combo pill: amiloride is what we call a potassium-sparing diuretic, and it's paired with hydrochlorothiazide specifically to help protect your potassium levels. So in theory, apple cider vinegar pulling potassium down isn't usually a big worry here, because half of your medicine is already working to hold potassium in.

That said, this concern is based on a single case report and is mostly theoretical, so the evidence is weak. A little vinegar on your salad won't matter. If you're swigging concentrated ACV daily, it's worth a quick chat with your pharmacist or doctor. Tell them if you notice muscle weakness, cramps, an irregular heartbeat, or unusual tiredness, since those can signal a potassium problem.

What Is Amiloride and Hydrochlorothiazide Used For?

Brand names: Moduretic, Amilzide, Moduret 25, Amil-Co

Amiloride hydrochloride and hydrochlorothiazide tablets are used for patients with high blood pressure (hypertension) or congestive heart failure who tend to develop low potassium (hypokalemia) when taking a thiazide or other diuretic alone — or for whom keeping potassium levels in a normal range is especially important, such as people taking digoxin or those with significant heart rhythm problems.

The tablets may be used on their own or alongside other blood pressure medicines, such as methyldopa or beta blockers. This combination product is not intended as the first-choice treatment for swelling (edema) or high blood pressure unless low potassium is a specific concern.
