Bitter Melon Ingredients & Drug Interactions
What is this page for?
First and foremost: checking Bitter Melon against your medications. The heart of this page is the interaction checker and the full interaction report — how this product’s ingredients may interact with prescription and over-the-counter medicines you may be taking.
Around that, we add a pharmacist’s high-level view of the product as a whole — what’s inside, the evidence for its stated use, how transparent the label is, and what safety data exists — so you can see the full picture in one place. It’s educational information from our licensed clinical databases and the clinical staff at HelloPharmacist — not medical advice — and we don’t sell or endorse products. Our editorial policy

HelloPharmacist Scorecard of Bitter Melon by Bixa Botanical
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: Momordica charantia extract, the plant compound from bitter melon. The capsule itself is made from HPMC, a plant-based capsule material.
Does it work?
Not established
The evidence we hold for bitter melon doesn't establish that it works for any of the conditions it's marketed for — amenorrhea, athletic performance, liver disease, constipation, diabetes, or dyspepsia all carry a rating of insufficient reliable evidence. That doesn't mean it doesn't work; it means the studies aren't solid enough yet for us to say.
How safe is it?
Well-documented data
Bitter melon taken by mouth is generally well tolerated, but be aware of the common side effects: heartburn, nausea, vomiting, diarrhea, constipation, flatulence, abdominal discomfort, dizziness, fatigue, and headache. These happened in about 3 to 16 percent of people in studies.
Rarely, very low blood sugar (hypoglycemia) or seizures (in children) have been reported. One case report documented kidney inflammation after months of high-dose bitter melon extract, which improved once the supplement was stopped.
In pregnancy, bitter melon is traditionally avoided because seeds and high doses may cause uterine contractions or other harmful effects — the safety data advises against it. While breastfeeding, there isn't enough reliable safety information, so talk with your doctor or pharmacist before using it.
Meds to double-check
Moderate interaction found
Check your exact medications before starting, especially antidiabetes drugs (metformin, insulin, sulfonylureas, and others) — bitter melon can lower blood sugar and may push it too low when combined. Also double-check if you take any P-glycoprotein substrates or pazopanib, as bitter melon may raise their levels.
The bottom line
Scorecard at a glanceFully disclosed formula with no established evidence rating for its marketed use. Moderate medication interactions have been identified, and safety information is well characterized.
Bitter melon may be worth considering if you're interested in blood sugar support, but the evidence for it isn't established yet in our data. If you take any diabetes medication or certain cancer drugs like pazopanib, or if you're pregnant or nursing, check with your pharmacist first.
The main risk is low blood sugar when combined with diabetes drugs.

Bitter Melon by Bixa Botanical Drug Interactions
HelloPharmacist Interaction Report
Bixa Botanical's Bitter Melon contains a single active ingredient — Momordica charantia extract — and it does interact with several medication types.
The most serious concern is with antidiabetes drugs (like metformin, insulin, or sulfonylureas): bitter melon can lower your blood sugar on its own, so taking it with diabetes medication raises the risk of low blood sugar (hypoglycemia). You'll need to monitor your blood glucose closely if you use both.
Read the full breakdown — every affected drug type, severity by severity
Bitter melon may also increase levels of P-glycoprotein substrates — a group of drugs that depends on a intestinal pump to move through your body — potentially making those medications stronger and raising the risk of side effects. Additionally, there's a case report of a patient on pazopanib (a cancer drug) who developed acute pancreatitis days after starting bitter melon juice, so theoretically bitter melon might raise pazopanib levels and increase its adverse effects.
Altogether, these interactions span 275 individual medications.
We hold no monograph data for any inactive ingredients in this product. Before you start, use the medication checker on this page to look up your exact drugs — especially if you take diabetes medication or any P-glycoprotein substrate.

Momordica charantia extract
Antidiabetes Drugs
Taking bitter melon with antidiabetes drugs might increase the risk of hypoglycemia.
Bitter melon can lower blood glucose levels and might have additive effects when used with antidiabetes drugs. This might increase the risk of hypoglycemia in some patients. Monitor blood glucose levels closely.
P-Glycoprotein Substrates
Theoretically, bitter melon might increase levels of P-glycoprotein substrates.
Bitter melon might inhibit the p-glycoprotein (P-gp) intestinal pump and increase intracellular levels of P-gp substrates. In vitro research in intestinal cells shows that 1-monopalmitin, a constituent of bitter melon, increases levels of daunomycin, a P-gp substrate. Additionally, drinking bitter melon juice has been associated with a case of acute pancreatitis in a patient who had been taking pazopanib, a P-gp substrate, for 8 years. Researchers theorize that inhibition of P-gp led to increased levels of pazopanib, resulting in pazopanib-induced pancreatitis.
Pazopanib (Votrient)
Theoretically, bitter melon might increase levels of pazopanib, potentially increasing the risk of adverse effects.
In one case, a 65-year-old patient taking pazopanib for 8 years for renal cell carcinoma experienced signs and symptoms consistent with acute pancreatitis 4 days after drinking bitter melon juice at a dose of 100-150 mL daily. The patient's symptoms, amylase levels, and lipase levels improved upon discontinuation of bitter melon and pazopanib. Pazopanib treatment was re-initiated with no further evidence of pancreatitis. Researchers theorize that inhibition of P-glycoprotein by bitter melon led to increased levels of pazopanib, a P-glycoprotein substrate, resulting in pazopanib-induced pancreatitis.
