How safe is it?
Well-documented data
Corn silk and marshmallow are generally well tolerated for short-term use, but long-term safety data are limited. The most common side effects from corn silk are low potassium and low sodium (hypokalemia and hyponatremia), especially with prolonged use or if combined with diuretics.
Uva ursi is safe at low doses short-term but can be toxic at high doses; at very high doses (30 grams or more), it may be life-threatening. It typically causes stomach upset, nausea, vomiting, and diarrhea.
Couchgrass (wheatgrass) is generally well tolerated but quality and contamination concerns warrant caution; it's also a major food allergen for wheat-sensitive individuals. Yarrow can cause skin reactions, dermatitis, and rarely airway reactions when handled topically.
Pregnancy: the safety data advise against corn silk, marshmallow, and uva ursi during pregnancy; yarrow is traditionally avoided because it may affect the uterus. Breastfeeding: safety data aren't sufficient for any of these ingredients, so speak with your doctor or pharmacist before use while nursing.

The most serious concern is with lithium — three of the ingredients (marshmallow, uva ursi, and yarrow) may theoretically reduce its excretion and raise its blood levels, which could require a dose adjustment to avoid toxicity.

Corn silk extract poses Moderate interactions with blood pressure drugs (antihypertensives), diabetes medications, diuretics, and blood thinners like warfarin. It may lower blood pressure and blood sugar, and contains vitamin K, which can interfere with warfarin if intake varies day to day.
It also increases potassium loss, which is a concern if you're taking corticosteroids. Couchgrass extract (wheatgrass) may lower blood sugar further with diabetes drugs and may affect how your liver breaks down certain medications (CYP1A2 substrates).
Uva ursi extract interacts with several drug-processing pathways in your liver (CYP2C19 and CYP3A4 substrates) and with glucuronidated drugs; these are Moderate interactions in theory, though not yet seen in humans.
Marshmallow extract carries a Minor interaction with general oral medications — its mucilage may slow absorption, so space it 30–60 minutes apart from other drugs. Altogether, these interactions span 2,127 individual medications.

certified organic Marshmallow extract
Lithium
Theoretically, due to potential diuretic effects, marshmallow might reduce excretion and increase levels of lithium.
Marshmallow is thought to have diuretic properties. To avoid lithium toxicity, the dose of lithium might need to be decreased when used with marshmallow.
Anticoagulant/Antiplatelet Drugs
Theoretically, marshmallow flower might have antiplatelet effects.
Animal research suggests that marshmallow flower extract has antiplatelet effects. However, the root and leaf of marshmallow, not the flower, are the plant parts most commonly found in dietary supplements. Theoretically, use of marshmallow flower with anticoagulant/antiplatelet drugs can have additive effects, and might increase the risk for bleeding in some patients.
Oral Drugs
Theoretically, mucilage in marshmallow might impair absorption of oral drugs.
Marshmallow contains mucilage which can affect oral drug absorption. To avoid changes in absorption, take marshmallow 30-60 minutes after oral medications.
certified organic Uva Ursi extract
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, uva ursi may decrease the metabolism of CYP2C19 substrates.
In vitro, uva ursi appears to inhibit cytochrome CYP2C19. This effect has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.
Glucuronidated Drugs
Theoretically, uva ursi may increase levels of drugs metabolized by glucuronidation.
In vitro, uva ursi extract appears to strongly inhibit UDP-glucuronosyltransferase (UGT) 1A1 (UGT1A1). However, uva ursi extract does not appear to inhibit UGT1A1 in animal models. This effect has not been reported in humans.
Lithium
Theoretically, uva ursi may increase lithium levels, necessitating a decrease in dose.
Uva ursi may have diuretic properties. Diuretics may increase lithium reabsorption with sodium in the proximal tubule of the kidney. Theoretically, uva ursi might reduce excretion and increase levels of lithium.
Urinary Acidifying Agents
Effects of uva ursi in the urinary tract may be reduced by urinary acidifying agents.
Uva ursi seems to work best in alkaline urine. Theoretically, taking uva ursi with medications known to acidify the urine may decrease any effects of uva ursi on the urinary tract.
P-Glycoprotein Substrates
Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.
certified organic Cornsilk extract
Antidiabetes Drugs
Theoretically, taking corn silk with antidiabetes drugs might increase the risk of hypoglycemia.
Animal research in diabetic mice shows that taking corn silk extract lowers fasting blood glucose levels.
Antihypertensive Drugs
Taking corn silk extract with antihypertensive drugs might increase the risk of hypotension.
Clinical research in both hypertensive and normotensive adults shows that taking corn silk extract lowers systolic and diastolic blood pressure.
Corticosteroids
Taking corn silk with corticosteroids might increase the risk of hypokalemia.
Clinical research shows that taking corn silk extract increases the urinary excretion of potassium.
Diuretic Drugs
Taking corn silk with diuretic drugs might increase the risk of adverse effects such as hyponatremia and hypokalemia.
Clinical research shows that taking corn silk extract increases urine volume and promotes the urinary excretion of sodium and potassium. Some patients may require electrolyte supplementation.
Warfarin (Coumadin)
Theoretically, suddenly stopping, starting, or changing corn silk treatment may alter the effects of warfarin.
Corn silk contains vitamin K. Individuals taking warfarin should consume a consistent daily amount of corn silk to maintain consistent anticoagulation.
certified organic Couchgrass extract
Antidiabetes Drugs
Theoretically, taking wheatgrass with antidiabetes drugs might lower blood glucose levels and increase the risk of hypoglycemia.
Animal research shows that taking wheatgrass stimulates the release of insulin from beta-cells and lowers blood glucose.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, wheatgrass might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that wheatgrass induces CYP1A2 enzymes.
certified organic Yarrow extract
Lithium
Theoretically, taking yarrow with lithium might increase the levels and adverse effects of lithium.
Animal research shows that yarrow has diuretic activity. Theoretically, due to these potential diuretic effects, yarrow might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.
