Apalutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the basic idea: your body relies on an enzyme called CYP3A4 to help clear apalutamide (Erleada). In lab studies, uva ursi seems to slow that enzyme down. If that happened in real life, apalutamide could build up a bit more than usual, which might mean a higher chance of its known side effects, like fatigue, joint pain, high blood pressure, skin rash, or rarely seizures.

The honest truth, though, is that this comes only from test-tube research and hasn't been seen in actual patients, so it's more of a theoretical concern than a proven problem. Apalutamide is an active drug, not a prodrug, so the worry is too much effect rather than too little. Still, since you're treating cancer, please run this by your oncologist or pharmacist before adding uva ursi.

What Is Apalutamide Used For?

Brand name: Erleada

Erleada (apalutamide) is used to treat two types of prostate cancer. The first is metastatic castration-sensitive prostate cancer (mCSPC) — prostate cancer that has spread to other parts of the body and still responds to hormone-lowering therapy. The second is non-metastatic castration-resistant prostate cancer (nmCRPC) — prostate cancer that has stopped responding to standard hormone therapy but has not yet spread to other organs.

Erleada is always used alongside a gonadotropin-releasing hormone (GnRH) analog — a medicine that lowers testosterone — or in patients who have already had surgery to remove the testicles.
