The culprit is pyrrolizidine alkaloids (compounds naturally present in the plant). This product should be a PA-free standardized form to minimize that risk, but even PA-free products carry a rare risk of liver injury.
The safety data advises against butterbur during pregnancy and while breastfeeding because the risks aren't well understood and liver safety is a concern.
