Bellafoline, Phenobarbital falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Butterbur interacts with.

Theoretically, CYP3A4 inducers might increase the toxicity of butterbur.
The pyrrolizidine alkaloid (PA) constituents found in some butterbur products are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

Here's the thing to understand about this pairing. The phenobarbital in Belladenal is a well-known liver enzyme booster. It revs up a system called CYP3A4, which is the same pathway that can turn certain butterbur compounds (called pyrrolizidine alkaloids, or PAs) into substances that are harmful to the liver. So in theory, taking phenobarbital alongside butterbur could speed up the creation of those toxic byproducts and raise the risk of liver injury.

The good news: this concern only applies to raw or non-purified butterbur. Quality products labeled PA-free have these troublesome compounds removed, so the worry largely goes away. I'd also note this interaction is theoretical, based on how the chemistry works rather than real reports. Still, please check with your pharmacist or doctor before combining them.
