Sotorasib falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Butterbur interacts with.

Theoretically, CYP3A4 inducers might increase the toxicity of butterbur.
The pyrrolizidine alkaloid (PA) constituents found in some butterbur products are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

Here's the twist with this pairing: it isn't really about your cancer drug building up or wearing off. Sotorasib (Lumakras) can speed up an enzyme in your liver called CYP3A4. That same enzyme is what processes the potentially harmful compounds (pyrrolizidine alkaloids, or PAs) found in some lower-quality butterbur products. So the worry is that sotorasib could ramp up the conversion of those PAs into toxic byproducts that are hard on the liver.

This is a theoretical concern based on how the chemistry works, not something proven in people, so I don't want to alarm you. The bigger safeguard is simple: only ever use a PA-free butterbur product. Since you're already on a serious cancer therapy, please run any butterbur use past your oncologist or pharmacist first.

What Is Sotorasib Used For?

Brand name: Lumakras

Lumakras is used to treat adults with KRAS G12C-mutated locally advanced or metastatic non-small cell lung cancer (NSCLC) who have already received at least one prior cancer treatment. This use was approved based on tumor response data, and continued approval depends on results from ongoing confirmatory studies.

Lumakras is also approved — used together with panitumumab — for adults with KRAS G12C-mutated metastatic colorectal cancer (mCRC) who have already received prior chemotherapy regimens containing fluoropyrimidine, oxaliplatin, and irinotecan. In both cases, patients must have a confirmed KRAS G12C mutation detected by an FDA-approved test before starting treatment.
