Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Butterbur interacts with.

Theoretically, CYP3A4 inducers might increase the toxicity of butterbur.
The pyrrolizidine alkaloid (PA) constituents found in some butterbur products are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

Here's the thing to understand about this pairing. Rifampin is one of the strongest CYP3A4 inducers we have, meaning it revs up that liver enzyme system. With most medications that speeds up their breakdown, but the worry here is different. Some butterbur products contain natural compounds called pyrrolizidine alkaloids (PAs) that can harm the liver, and CYP3A4 is what turns those PAs into their toxic form. So in theory, rifampin could ramp up that conversion and make liver injury more likely.

I want to be honest that this is a theoretical concern based on how the enzyme works, not on real patient reports. The simplest fix is to only use PA-free (certified) butterbur, which sidesteps the issue. Either way, please run it by your pharmacist or doctor before combining them.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
