Nevirapine falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Butterbur interacts with.

Theoretically, CYP3A4 inducers might increase the toxicity of butterbur.
The pyrrolizidine alkaloid (PA) constituents found in some butterbur products are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

Here's the thing to know about this pairing. Nevirapine is one of those HIV medications that revs up a liver enzyme called CYP3A4. Now, certain butterbur products still contain pyrrolizidine alkaloids (PAs), the natural compounds that can harm the liver. That same CYP3A4 enzyme turns those PAs into toxic byproducts. So in theory, by speeding the enzyme up, nevirapine could make any leftover PAs more damaging to your liver, not less.

I want to be honest that this is a theoretical concern based on how the chemistry works, not something proven in people. Still, nevirapine already carries its own real risk of liver problems, so stacking another liver stressor isn't ideal. The simplest safeguard is to only ever use a PA-free (PA-certified) butterbur product, and please run it by your pharmacist or doctor first.

What Is Nevirapine Used For?

Brand names: Viramune, Viramune XR

Nevirapine is used alongside other antiretroviral medicines to treat HIV-1 infection. It is not a cure, but it helps suppress the virus when used as part of a combination regimen.

The immediate-release tablet is approved for adults and pediatric patients 15 days and older. The extended-release tablet is approved for adults and for pediatric patients 6 years of age or older who meet a minimum body surface area requirement. The oral suspension is approved for adults and pediatric patients 15 days and older. None of these formulations should be used alone to treat HIV-1, and none are approved for post-exposure prophylaxis (PEP).
